Indian Health Service Special Diabetes Program for Indians 2023 Application Project Abstract American Indian Health and Family Services (AIHFS) is a designated Urban Indian Organization (UIO) delivering primary health care and integrated behavioral health and cultural services. We are located in Detroit Michigan and service a seven-county urban area, within the Bemidji Region, for IHS eligible and recognized Tribal members. Our Special Diabetes Program for Indians (SDPI), will focus on Best Practice in the areas of: Eye Exam – Retinopathy Screening and Blood Pressure. The focus on Blood Pressure <140/90 will be used to improve medication adherence through the use of at home blood pressure monitoring and the inclusion of Telemedicine services. Our Eye Exam best practice will provide enhanced care by implement and using RetinaVue screening in-house versus referring out and directly improving patient screening for Diabetic Terinopathy. Key measurement and supportive service activities to address our Best Practice goals include: Nutrition Education; Fresh Prescription Programming; educational handouts; Point-of-Care Lipid Screening; and Point-of-Care Microalbmin Screening. Our Target Group is AI/AN patients with a diagnosis of Type II Diabetes Mellitus or lab value indicating Pre-diabetes. The target group is further defined by our existing diabetic care patients, the goal is for the group to participate in all Best Practice goals and respective activities. Since 1978, American Indian Health & Family Services (AIHFS) has been a nonprofit health and community wellness center whose mission is to empower and enhance the physical, spiritual, emotional, and mental well-being of underserved populations in Southeast Michigan. The center’s approach integrates traditional Native American healing and spiritual practices with contemporary Western medicine in both treatment and prevention through the use of the Integrated Care Model. The SDPI program model proposed is based upon the establish best practices for diabetic care and integrated inclusion into all agency services.
