NATIVE HEALTH Special Diabetes Program for Indians:
NATIVE HEALTH, dba Native Health Community Health Center, Inc, the Title V – Urban Indian Health Provider in Phoenix, Arizona is applying under the Indian Health Service (IHS) Special Diabetes Program for Indians Community-Directed Grant Program. The total federal funding request that NATIVE HEALTH is seeking to secure in 2023 is $349,749.

Diabetes-Related Education
Diabetes education helps reduce the risk for developing diabetes and its complications Number and Percent of individuals in your target group who receive education* on any diabetes topic, either in group or individual setting: *nutrition education, physical activity education, any other diabetes education
Target Population
• 330 active diabetic patients
• 60 indigenous youth
• 300 community members and families at-risk
Proposed Activities/Services:
1) Diabetes Related Education – Delivered on an individual or group setting at the NATIVE HEALTH-Central location for American Indian Community members diagnosed at-risk, Pre-Diabetes, or with Diabetes.
a. Increase intensity DRE: 6-week Stanford Chronic Disease Self-Management Curriculum for active patients with Diabetes diagnosis.
2) Community Based Diabetes Related Education - DRE delivered at community venues in areas of notable American Indian populations. This includes school based DRE at local school districts, and the Living Well Traditionally Diabetes Prevention Camp.
3) Outreach – Outreach and Recruitment – Delivered in communities of notable American Indian presence, the program staff will promote NATIVE HEALTH and SDPI Programs and to increase client base. Multi-media advertisement campaign utilizing NH staff. Multiple outreach activities conducted per quarter.
4) Partnership – Increased community partnerships with Arizona school districts and Phoenix based non-profits serving American Indians and youth.
ESTIMATED NUMBER OF PEOPLE TO BE SERVED: 690 Target Population; 75,000 AI/AN of Maricopa County to receive outreach services.
