A treatment and prevention program for Type II Diabetes in the American Indian/Alaskan Native communities is very important. A successful program can be an effective tool to registry participants to gain the knowledge and resources to improve health outcomes. The goal of South Dakota Urban Indian Health, Inc. is to continue to serve the urban AI/AN community and implement a program that uses evidenced-based practices to provide quality health care. The proposed program will be implemented using the Diabetes Standards of Care with an integrated care team in conjunction with activities and services to allow the participants to successfully self manage their health.
