Urban Indian Center of Salt Lake will provide diabetes management and prevention services to AI/AN in the Wasatch Front area. The Diabetes Best Practice will be "Glycemic Control," focusing on increasing the number and percent of persons with diabetes who receive medical care at UICSL and have A1c less than 8.0%. Other activities will be focused on diabetes prevention, including the NDPP, and diabetes awareness among community members. This will be achieved through diabetes screening, community activities, physical fitness programs, and food and nutrition programs. UICSL staff include medical personnel, a Registered Dietitian, Certified Personal Trainer, and NDPP Lifestyle Coaches.
