SDPI FY 2023 project includes treatment of diabetes health related issues for all program participants - The Fort Bidwell Indian Community, Warner Mountain Indian Health Program (WMIHP), is a small Native American Clinic located in the very most Northeast corner of California. The WMIHP Clinic is located on a reservation that sits in an extremely remote area creating a hardship for residents to access healthy, fresh, and affordable food.
The WMIHP Clinic has almost a 20% diabetes prevalence amongst its Native American/Alaskan Native user population. The high prevalence of diabetes amongst our Native American community is the reason why The WMIHP Clinic uses a proactive Team-Based Care approach to provide quality meaningful medical care and patient education.
Our Team consists of a Medical Doctor specializing in internal medicine, a Registered Nurse, a Community Health Representative and supporting staff. Our team will focus on diabetic best practices to target key areas of diabetic care for all patients. Once a best practice is chosen, the team will coordinate patient care by planning specific diabetic clinic dates to address each patient and areas of care.
Through our participation in submitting data to IHS Diabetes Care and Outcomes Audits we can identify Key diabetes-related health issues specific to our community. Obesity, high blood pressure, cardiovascular disease, lack of physical activity, A1c's >9, and metabolic syndrome are health issues identified and require additional attention.
To address these key diabetes-related health issues, the team will plan monthly diabetes care and community education meetings to help patients and their families understand diabetes and the benefits of choosing healthy eating, exercise, and diabetic self-care. Diabetic care incentives will be used to encourage patients to choose healthy living practices that will benefit their diabetic outcomes.
Community meetings with our diabetic patients and families will include question and answer sessions with our Medical Doctor and Registered Nurse. Our diabetic patients and families will also participate in planning monthly topics of education and healthy lifestyles to help expand not only our diabetic patients but also our community members knowledge of diabetes.
The goals of the Special Diabetes Program for Indians, team approach to addressing the high prevalence of diabetes in our small rural community, is to encourage our diabetic patients and community members to choose healthy lifestyles. We hope to accomplish this by increasing their knowledge about diabetes. This shared knowledge will assist in our efforts to decrease the key diabetes-related health issues within our community and positively increase our best practice outcomes for our diabetic patients.
