The diabetes program is under the Healthy Traditions department, this department is mainly supported by SDPI funds. Healthy Traditions strives to work with all the departments located within Sonoma County Indian Health Project, Inc. to provide high quality diabetes services to our AI/AN communities. HT is an accredited (AADE) program that offers in collaboration with the medical department, diabetes services ( e.g. foot exams, retinal photographs, and A1Cs) and education (individual counseling and group classes).
HHS’ Tracking Accountability in Government Grants System (TAGGS) website provides access to detailed descriptions of grants, loans, aggregated direct payments and other types of financial assistance awarded by the HHS Operating Divisions (OPDIVs) and the Office of the Secretary Staff Divisions (STAFFDIVs).
