The proposed project aims to improve Type 2 diabetes control and prevention among the identified American Indian Alaska Native (AIAN) population with a comprehensive approach. The proposed project focuses on targeted outreach and treatment which will include: assessment and case management; monthly diabetic clinics; health fairs and wellness activities. The best practices that will be addressed and tracked include influenza immunizations, blood pressure management, chronic kidney disease management, and physical activity and nutrition. To ensure inclusivity and equity among communities, the proposed project will be developed with input from the tribes and families and will include culturally sensitive activities that involve language, food, music, and spirituality.
