KLAMATH TRIBES
The Klamath Tribes is applying for 2023-2028 Competitive grant funding of the Special Diabetes Program for Indians. The SDPI Grant funding has been available through Indian Health Services since 1997. The Klamath Tribes has been receiving this funding since 2007. The Klamath Tribes Wellness Clinic is located in Chiloquin, OR and serves all Native American/Alaska Native persons who are enrolled in federally recognized tribe(s). This funding will be administered by the Diabetes Management Program under the supervision of the Public Health Department Director and Supervisor. The goals and objectives that this proposal seeks to address are: creatively delivered researched based education on diabetes prevention and diabetes care and self-management, cooking and nutrition classes, community walk/run events, health fairs for men, women, elders, nutritional counseling, Podiatry care, annual diabetic eye exams, and incentives to encourage healthy activities and lifestyle. The oversight for the SDPI Grant for The Klamath Tribes is professionally managed by a grant coordinator, annual diabetes audit supported by Indian Health Services, North Portland Area Indian Health Board, Klamath Finance Health Officer and annual financial audit from the State of Oregon. SDPI receives support from the Klamath Tribes’ Health General Manager, and support by Klamath Tribal Council’s Tribal Resolution. SDPI is locally supervised by the Public Health Supervisor and the Director of Public Health. The SDPI Diabetes Program partners with up to 18 health agencies in Klamath Falls and Chiloquin, Oregon and also shares best practices on a monthly basis with the 9 Tribes of Oregon through the Diabetes Echo Program via an online platform. Klamath Tribal Health’s Medical Clinic and its Public Health Department team are committed along with Tribal Administration, Tribal Council and other community agencies, to bring culturally appropriate events, medical care, and education in classes to tribal members o
f all ages with the aim to prevent diabetes and to manage gestational, Type I, and Type II Diabetes.
