To provide diabetes clinical and prevention services to eligible participants within the boundaries of the Pyramid Lake Paiute Tribe Reservation. - The Pyramid Lake Indian Reservation is located in Washoe County, Northern Nevada, approximately 30-45 miles from Reno-Sparks, Nevada. There are 3 locations; Wadsworth, Nixon and Sutcliffe. The Pyramid Lake Tribal Health Clinic is located in Nixon-17 miles from Wadsworth and 17 miles from Sutcliffe. To access many specialty medical services the patients must travel to the Reno-Sparks area.Transportation is available via the Tribal Transit Program, the bus runs daily through the communities and to the Reno-Sparks area. Transportation is also available from the Pyramid Lake Tribal Health Clinic and also the Diabetes Wellness Program. When we have had physical activities in the past we schedule them at the Wadsworth and Sutcliffe Community Centers. In Nixon, we try to schedule activities at any space that is available. We desperately need a Diabetes Wellness Center. The Diabetes Wellness Program is situated in the Pyramid Lake Tribal Health Clinic with little space. The prevalence of diabetes on the Pyramid Lake Indian Reservation is 8.1%. WE have identified Foot Exams as our Best Practice because the statistics and morbidity issues have been increasing severely in the last 4 years. We have had 5 amputations in the last 4-5 years and we had had no amputations or very few from 2007-2014. WE have 5 ESRD patients, receiving dialysis 3 times a week. We need to work with patients earlier before they get to that point. Due to the pandemic our exercise, nutrition, and diabetes education programs had become few. Most of the diabetes education has been provided one on one with the medical providers. There are 3 locations, Wadsworth-3 miles from Fernley, Nevada where food shopping is available; Nixon-20 miles from Fernley, Nevada where food shopping is available and Sutcliffe-25 miles from Spanish Springs, Nevada where food shopping is available. Due to Covid-19 many of the community gardens, and Senior garden were discontinued. The Farmer's markets in the Reno-Sparks area are now beginning to open, although they are 25-45 miles away. The Pyramid Lake Paiute Tribe has a food distribution program open daily with deliveries to Covid-19 The Pyramid Lake Diabetes Wellness Program is basically a clinical program and exists within the Pyramid Lake Tribal Health Clinic. The diabetes patients on the Diabetes Registry receive their medical services onsite. We have several contractors that provide: Chiropractic services, Podiatry services, RN CDE services, Dental services, and Optometry services. During the pandemic many services were limited due to Covid facility standards. We are slowly getting back to normal, however, many of the services are being provided in designated pods rather than within the facility.
