PROBLEM: In our communities we have an increase of obesity, elevated blood pressure and elevated A1-C glucose levels. This increase is due in part to the COVID-19 restrictions which included being home bound, lack of exercise, limited medical visits and food insecurities.
TARGET POPULATION: Current diabetic population as recorded in our Electronic Health Record (EHR),are pre diabetic and those who have a family history of diabetes.
GOALS: Reduction of obese patients Body Mass Index (BMI),lower blood pressures and A1-C at target of <8
MEASUREMENTS: The program is focusing on the three (3) main areas we consider most serious being obesity,high blood pressure and elevated A1-C. The measurements will be monitored by Electronic Health Records (EHR), nutritional consulting and attending physical fitness activities.
PLAN OF ACTION: We intend to address these issues by the following means
1- Providing weekly nutritional classes with our Registered Dietitian, along with cooking classes, and healthy food choices. We also will be having our hoop house/tunnel house in full production which will give fresh produce to our tribal members. This in turn will help eliminate some food insecurities for our diabetic population.
2-Increase hours of operation of our Wellness Center with three (3) Certified personal trainers. Increasing incentives for working out and other wellness activities. These include a Health Fair and three(3) fun runs through out the year.
3-Provide glucose monitoring equipment, blood pressure cuffs, athletic shoes for walking, dental appliances for better nutrition and eye glasses.
