Fort McDowell Yavapai Nation (FMYN)is a rural Indian tribal community located near Phoenix, Arizona. FMYN has a centrally situated Wassaja Memorial Health Center (WMHC), a medical clinic that provides family practice services to the tribal members. Close to 1000 community members receive healthcare from WMHC. Housed in WMHC is the Community Health division which provides preventative health services, including the administration of SDPI program activities. FMYN has been a recipient of the Special Diabetes Program for Indians (SPDI) grant since the early 2000. FMYN has been an active recipient of the SDPI grant since that time. For the current 2022 year, our SDPI program staff have been providing 'Diabetes-related Education' as the best practice to render diabetes prevention and/or treatment education activities and services to our tribal members. The SDPI program at FMYN is titled, 'Healthy Futures'. For the 2023 SDPI program, we wish to continue with the same best practice to build on the progress made from the current year. Using the Diabetes Audits from 2020 and 2021, we gathered data that suggests a high incidence of obesity, hypertension, and hyperglyceridemia among our diabetes patients. We are preparing a WMHC clinical protocol for the calendar year 2023 to funnel all diabetes patients with obesity, hypertension, and high serum triglycerides to our nutrition specialist for an intensive one-on-one nutrition counseling and to our fitness specialist for customized physical activity programs. In addition, we are using our SDPI program staff strengths and professional experience for implementing the following major activities: provide one-on one nutrition counseling and physical fitness counseling for patients at-risk for pre-diabetes and diabetes; provide diabetes education presentations to all members of the community; conduct several "healthy cooking" demonstrations through the year; and organize "diabetes
care days" to implement diabetes management protocols to all patients with diabetes. Several other diabetes-prevention activities will be provided through out the year to bring awareness of diabetes in our target group. We estimate our target group to be around 250 tribal members.
