The Fort Defiance Indian Hospital Board, Inc. (FDIHB, Inc.) Diabetes Program has been funded by the Special Diabetes Program for Indians for more than 10 years. The program serves the Navajo population in 16 communities located on the Navajo Reservation in the Southwest United States, covering 2,200 square miles. The total population in the Fort Defiance Service area is 39,804 patients that are active and have been seen by a primary care provider within the last two years, the majority being Navajo tribal members. FDIHB, Inc. is a PL93-638 self-determined tribal health care organization. The Diabetes Program operates under the Medical Division at FDIHB, Inc.
The Diabetes Program provides services and education to support self-management and prevention of diabetes to improve overall health status to reduces incidence, prevalence and premature death related to diabetes.
The Fort Defiance Indian Hospital Board, Inc. (FDIHB, Inc.) Diabetes Program has an interdisciplinary team approach that coordinates care and collaborates to deliver safe, effective and quality services that aim to support our staff by enhancing their knowledge and skills to provide competent and evidence-based teaching to our diabetes population that we serve.
The Diabetes Program provides education individually in the outpatient and inpatient setting, in a group setting, in the community service areas and through partnerships with outside entities. Diabetes awareness is a collaborative team effort which all departments unify and facilitate health fairs, health events and making public announcements that aims to engage all community members in the FDIHB, Inc. service areas.
The Diabetes Program and the interdisciplinary departments aim to work collaboratively by supporting patients to improve their health by managing or preventing diabetes through a patient-centered, culture-sensitive approach.
Services, activities and interventions have been implemented successfully with the support of the SDPI Grant Funding. The FDIHB, Inc. Diabetes Program appreciates the assistance we receive to improve the quality of life for our Dine people that leads to Hozho.
