The Alabama-Coushatta Tribe of Texas and the Chief Kina Clinic is seeking support of its diabetes care and prevention program through funding announcement HHS-2023-IHS-SDPI-0001. The health clinic along with Special Diabetes Program for Indians will engage in providing the tools necessary to our Tribal patients, community and their families to prevent and reduce the onset of diabetes. There are approximately 762 users of the Tribal health facility of which 152 are diagnosed with diabetes. The goal of this project is to increase prevention and reduce the onset of diabetes as well as reduce other chronic diseases associated with diabetes. Over the course of the project, the Alabama-Coushatta Tribe of Texas, Chief Kina Health Clinic and its partners will ensure that diabetes care and prevention issues are addressed, implemented, coordinated, as well as ensure support of training in the area of patient-focused services with the use of clinic staff. It will be a priority to work with other Tribal programs that provide services to the Tribal patients, community and families in implementing a comprehensive community prevention/early intervention program for diabetes, and initiate the process of creating an action plan for their health and well-being.
