Tsitewatakari:tat- The Let’s Get Healthy Program is part of the Saint Regis Mohawk Tribe (SRMT) Health Services. We are located on the Akwesasne Mohawk Territory in New York State. Through individual, family and community services, The Let’s Get Healthy Program is committed to providing excellence in diabetes care and prevention. We are seeking funding of the Special Diabetes Program for Indians Community-Directed Grant. We plan to provide Diabetes-related Education as our Best Practice, including individual appointments and group classes. On a daily basis our program provides support through personalized case management with our Nurse, virtual nutrition counseling appointments with a Registered Dietician through referral, and exercise programs to meet the needs of our participants. With a state-of-the-art fitness room at the Diabetes Center for Excellence, we are able to offer various fitness programs for adults and an after-school youth class targeting those at risk for diabetes. We've fostered many community partnerships which enable us to provide educational outreach to families, youth and seniors for diabetes prevention and to promote healthy living. A diabetes epidemic exists within Indian Country. The burden of diabetes in the American Indian/Alaska Native population is greater than other racial and ethnic groups. The prevalence rate for SRMT is 16% compared to 11% for the IHS-wide user population (2016 IHS Diabetes Care and Outcome Audit). Providing Diabetes-related Education for our Native American community will allow for improved awareness and knowledge to encourage healthy behavior change. Behavior change strategies can then lead to prevention of diabetes and diabetes related complication
