Diabetes is one of the most prevalent diseases afflicting the Wind River Indian Reservation, home of the Northern Arapaho and Eastern Shoshone tribes. According to Wind River Family and Community Healthcare 2021 Diabetes Audit, 56% of tribal members are diagnosed with diabetes or are at risk of developing diabetes. Our 2022 Diabetes Audit indicates out of our diabetic population 23% have a Hbg A1C of 9.0 or greater, 53% are obese, 65% are diagnosed with hypertension, 36% diagnosed with cardiovascular disease, and 21% diagnosed with retinopathy. The Northern Arapaho Diabetes Prevention program (NADP) has worked to address these morbidities throughout the past 23 years. NADP focuses on community needs through offering diabetes education and lifestyle management assistance to community members. The program’s mission is to empower our participants with self-care management skills such as physical health awareness and through referring clients to pertinent resources and support networks. The goal for teaching these skills and offering services is to improve participant quality of life and lessen the burden of diabetes. NADP serves 899 diagnosed diabetics and over 4,000 community members annually. The program manages two public fitness centers, offers evidence-based lifestyle balance courses, and diabetes case management. Northern Arapaho Diabetes Prevention program’s goal is to educate and offer services to all age groups within the community. NADP will continue to encourage good health and wellness and raise diabetes awareness through diabetes education in 2023. The program’s focus will be to engage in diabetes prevention education starting with youth age groups and continuing to support elder age groups with general health morbidity management. The program’s education efforts will coincide with diabetes case management for current diagnosed participants and outreach to at-risk populations. Case management will include assistance with appointment sc
heduling and referrals, and assistance with eyeglasses, dentures, and proper fitting shoes.
