Ryan White Title IV Women, Infants, Children, Youth and Affected Family Members AIDS Healthcare - Address: Arleta, California (Serving the entirety of Los Angeles County) Project Director: Navid Kahen Total Annual Funding Requested: $610,000 So-Cal Community Health Center (So-Cal), a Federally Qualified Health Center Look-Alike located in Arleta, California, requests Ryan White HIV/AIDS Program Part D funding under HRSA-26-067 to establish an integrated, family-centered medical home for low-income, uninsured, underinsured, and culturally diverse women ages 25 and older, infants ages 0–2, children ages 2–12, youth ages 13–24, and affected family members living with or affected by HIV throughout Los Angeles County. So-Cal currently serves approximately 5,000 unduplicated patients through 15,800 annual visits. The proposed project will enroll approximately 140 unduplicated participants annually. Project Director: Navid Kahen. Annual Funding Requested: $610,000. Women, infants, children, and youth affected by HIV in Los Angeles County encounter significant structural, financial, linguistic, cultural, and healthcare-system barriers that interfere with timely diagnosis, linkage to care, retention, treatment adherence, and viral suppression. Poverty, housing instability, transportation limitations, stigma, fragmented specialty services, and difficulties transitioning from pediatric to adult care place vulnerable families at elevated risk of delayed or interrupted treatment. Using a “no wrong door” approach, eligible participants will enter the program through So-Cal’s fixed-site primary care, obstetric and gynecological services, pediatric services, HIV testing, hospital and physician referrals, community partnerships, and mobile healthcare services offered during weekday, evening, and weekend hours. So-Cal will provide coordinated HIV primary medical care, medication management and adherence support, laboratory monitoring, women’s health, prenatal and postpartum care, pediatric monitoring, behavioral health services, health education, and specialty referrals. Support services will include non-medical case management, multilingual communication assistance, transportation coordination, basic-needs referrals, and linkage to community resources. Clinical leadership will include Dr. Pedram J. Khorsandi, Medical Director/Internal Medicine; Dr. Tanaz Kahen, Chief Medical Officer/OB/GYN; Dr. Yousef Mehrabi, Pediatrician; Dr. Carol R. Kiriakos, Psychiatrist; participating physician assistants; and supporting clinical and administrative personnel. So-Cal will use its Meditab integrated electronic health record and practice-management system to document enrollment, services, laboratory results, referrals, missed appointments, retention, and required Ryan White reporting data. Program goals include timely linkage to medical care, an 85 percent retention-in-care rate, viral suppression of at least 80 percent among enrolled clients, and prevention of perinatal HIV transmission. Performance data, patient feedback, referral outcomes, and clinical indicators will be reviewed through So-Cal’s Clinical Quality Management program. Root-cause analysis and Plan-Do-Study-Act cycles will be used to correct performance gaps, strengthen service coordination, and improve outcomes for WICY participants and affected families.
