MSMW served 818 Women, Infants, Children and Youth (WICY) living with HIV in 2018, 816 in 2019, and 747 in 2020. These services were provided in two clinic locations including: Morningside Clinic at Mount Sinai Morningside Hospital on 114th Street and Amsterdam Avenue, and the Samuels Clinic at Mount Sinai West Hospital on 59th Street and 10th Avenue. In 2013, St. Luke’s-Roosevelt became part of the Mount Sinai Health System, and the IAM was created to improve efficiencies and share best practices throughout the eight hospitals and clinics now affiliated. The number of WICY patients being served at MSMW has declined recently reflective of the decreases in the proposed service area (PSA), however, the community still has over 2,878 WICY with HIV, including 75 new diagnoses in 2020. Services: The PSA includes the following New York County neighborhoods: Washington Heights/lnwood (10031,10032,10033,10034,10040), Central Harlem (10026, 10027, 10030,10037,10039), East Harlem (10029,10035), Upper West Side (10023,10024,10025), Chelsea/Clinton/Greenwich Village (10001, 10011, 10012, 10013, 10014, 10
018, 10019,10020, 10036). MSSLW is requesting $897,048 for each year of the contract period 2022-2026. Services will be provided annually for 880 WICY with HIV in 2022-23, who are uninsured/underinsured, underserved, and vulnerable. The number of WICY with HIV to be served will increase to 962 in 2025-26. Key services will include: HIV primary/specialty medical care, oral health care, mental health, medical nutrition therapy, substance abuse screening/treatment, medical case management, transportation assistance, outreach, targeted testing/linkage to care, childcare, patient education, and tailored support groups and services. Part D supports MSMW’s Young Adult Program (YAP) to reach our young adults, similar to the evidence-informed intervention “Centralized HIV Services.” MSMW will implement a number of quality improvement projects that will assist us to monitor our success achieving positive health outcomes among the WICY population. These will include monitoring the number and percent of 1) WICY prescribed ARVs who have an undetectable viral load; 2) WICY retained in care; 3) women that have a gynecological evaluation; and 4) WICY screened, counseled and treated for tobacco use. Black women and youth will continue to be our populations of focus. Target Population: MSMW will serve an average of 920 WICY annually. This will include 28 youth ages 13-24 and 892 women 25 years old or older. Based on our current population served, we expect that more than 90% of will identify as Black or Hispanic and the majority will identify as behaviorally infected.
