(JCCSI) is a stable, 330 funded FQHC that provides primary care medical, dental, behavioral health, nutrition, education and HIV/AIDS services to urban and rural communities and homeless populations in central Arkansas. JCCSI was incorporated in 1972 and was the first FQHC in Arkansas to develop HIV treatment programs. For over 50 years, JCCSI has worked closely with community leaders to evaluate and respond to the health care needs of vulnerable and minority populations, particularly those with HIV/AIDS. In order to address HIV/AIDS and the psychosocial issues surrounding the disease that affect families, JCCSI defines its target population as HIV infected women over the age of 25, HIV infected and exposed infants, and HIV infected children and youth. As an FQHC, JCCSI utilizes a Patient Centered Medical Home (PCMH) model of care to directly provide these target populations with comprehensive, holistic, family-centered services at eight primary care clinic sites and a homeless clinic site, and JCCSI partners with other care delivery organizations to provide expanded HIV/AIDS services in the broader service area. Over the last project period, JCCSI served between 50-70 HIV infected WICY patients annually at its clinics and its subrecipients (East Arkansas Family Health Center, Cabun Rural Health Services, and Lee County Cooperative Clinic) have collectively served between 6-10 additional patients annually. Because of its expertise in identifying and treating the HIV/AIDS populations in Arkansas, JCCSI’s RWHAP Part D grant covers an extensive service area. Counties that are served with this federal funding include Arkansas, Ashley, Bradley, Calhoun, Chicot, Clark, Cleveland, Columbia, Crittenden, Cross, Dallas, Desha, Drew, Garland, Grant, Hempstead, Hot Spring, Howard, Jefferson, Lafayette, Lee, Lincoln, Little River, Lonoke, Miller, Mississippi, Monroe, Montgomery, Nevada, Ouachita, Phillips, Pike, Poinsett, Polk, Prairie, Pulaski, Saline, Sevier, St. Francis, Union and Woodruff. JCCSI partners with the Arkansas Health Department and 3 FQHCs throughout this expanded service area for testing, patient care, education, and other social services related to the comprehensive treatment plans needed to manage HIV/AIDS for the WICY population. JCCSI has a well-developed, comprehensive treatment program for WICY individuals who are either living with or exposed to HIV/AIDS. Included in this comprehensive treatment program are the following services: medical care, HIV Testing and education, housing referrals, laboratory monitoring, nutritional counseling, medication assistance, transportation assistance, vision care referrals/assistance, onsite dental services at JCCSI dental clinics, dental care referrals/assistance for patients who cannot access JCCSI’s dental clinics, case management, legal referrals/assistance, utility assistance, onsite substance abuse screening at JCCSI medical clinics, referrals to HIV clinical trials, referrals to RWHAP Part B services, and patient advocacy. In addition to the extensive treatment and supportive services offered through the RWHAP Part D funding, JCCSI has a robust quality program – The Ryan White Program Wide Quality Management Plan - to measure and monitor treatment success.
