Access to Healthcare: covering the day-to-day expenses of our transportation service, allowing us to continue offering free or low-cost rides for patients who would otherwise struggle to access care. - Access to Healthcare is a transportation assistance initiative designed to reduce one of the most significant barriers to healthcare access in rural communities: reliable transportation. Through this program, Uncompahgre Medical Center provides free or low-cost rides for patients who would otherwise face difficulty attending medical appointments due to geographic isolation, limited transportation options, financial hardship, age, disability, or mobility challenges. Grant funding will support the day-to-day operational expenses of the transportation service, including personnel, fuel, vehicle maintenance, insurance, registration, and outreach activities. These resources ensure that patients can access primary care, specialty services, preventive screenings, behavioral health appointments, and other essential healthcare services in a timely manner. By removing transportation barriers, the program helps reduce missed appointments, improve continuity of care, support chronic disease management, and prevent avoidable emergency department visits and hospitalizations. The initiative primarily serves rural and underserved populations throughout the West End of Colorado, where long travel distances and limited public transportation significantly impact healthcare access. Ultimately, the project promotes continuity of care by ensuring that transportation challenges do not prevent individuals from receiving the care. Through sustained transportation support, Uncompahgre Medical Center aims to improve health outcomes, increase healthcare utilization, and strengthen the overall well-being of the communities it serves.
