No Brief Description of Target Population: The target population for this grant are residents of Cleveland County NC with a focus on pregnant and newly parenting mothers (11.2% of babies in the county are born to mothers with SUD), people with acute Hepatitis C (4/100,000 county residents has acute Hepatitis C vs. NC average of 1.6/100,000), the uninsured, many of whom are racial and/or ethnic minorities (19.7% of all adults in county uninsured; 24.7% of African American and 64% Hispanic adults uninsured), people with low incomes (21.3% of people in Cleveland County have incomes below 100% federal poverty), DSS-involved families (10.7% of newborns engaged in Care Coordination for Children program affected by substance use as a result of Plan of Safe Care referral vs. 3.7% NC average). A very small percentage of the Cleveland County population (0.2%, 289 people) is Native American.
