Brief Description of the Target Population - Indicate approximately what percentage (if any) of the target population is American Indian/Alaskan Native; The Mat-Su borough has a population of 107,081 residents, a land and water area of over 25,000 square miles, and a population density of 4.3 residents per square mile. Around 11% of Mat-Su borough residents are Alaska Native and/or American Indian (race alone or in combination). The consortium includes 2 Tribal social services providers and 2 federally recognized Tribes. Among other things, the project will implement culturally and linguistically appropriate approaches for Alaska Native, American Indian, and Hispanic individuals. The consortium includes 1 AIDS support and resources organization and 1 youth homeless shelter and resources center. Via those consortium members, the project will serve people who have AIDS and people who are homeless. 14. Target Service Area (must be exclusively rural, as defined by the Rural Health Grants Eligibility Analyzer) - Fully Rural Counties: Provide the county name and state - Partially-Rural Counties: Provide county name, state, and the rural census tract (list of rural census tracts) Matanuska-Susitna (Mat-Su) borough in Alaska.
