Rural Communities Opioid Response-Implementation - Countering the SUD and OUD Epidemic (CSOE) Ampla Health 935 Market Street, Yuba City, CA 95991 Cindy Snelgrove, Chief Clinical Services Officer 530-674-4261 ext. 1155 csnelgrove@amplahealth.org https://www.amplahealth.org/ Funds requested: personnel, equipment, supplies, travel, and contractual Project Abstract: Ampla Health, with its Corporate Office located at 935 Market Street, Yuba City, CA 95991, is a 501 (c) 3 non-profit network of community based Federally Qualified Health Centers (FQHC) in Northern California offering health care services in Butte, Colusa, and Glenn counties, is seeking $1,000,000, with a consortium commitment with other county resources, to fund the “Countering the SUD and OUD Epidemic (CSOE) project.” Under the direction of Cindy Snelgrove, Chief Clinical Services Officer (CCSO) (530-674-4621 ext. 1155, csnelgrove@amplahealth.org) Learning of the Funding Opportunity: HRSA-22-057 by way of the HRSA website, and not being a previous RCORP award recipient, Ampla Health’s CSOE will provide access to prevention, intervention, treatment, and recovery resources/services for individuals where are at risk for, have been diagnosed with, and/or are in treatment and/or recovery for SUD or OUD who reside in HRSA-designated rural areas. The target population is 3,758 current patients, 20-75 years of age that have been diagnosed and /or prescribed opioids on the electronic health record (EHR) of the 84,195 unduplicated patients Ampla Health provides health care services in Butte, Colusa, Glenn, and Tehama counties, as well as historical populations suffering from poor health outcomes and health disparities such as homelessness, racial, and ethnic minorities, pregnant, LGBTQ individuals, the elderly, individuals with disabilities, etc., including at-risk populations of patients living with HIV/AIDS, youth at risk for depression and not treated (32%), individuals using opioids for pain management, and individuals lacking access to MAT services including linguistic and culturally diverse residents. The targeted service areas do not overlap with an existing FY 19 OR FY 20 RCORP-Implementation award. In collaboration with a consortium, including the outcome in leveraging partnerships and ensuring linkages to supportive services, coordination, treatment, intervention, and recovery service will be attained and health equity restored. Collaborative consortium members, including Ampla Health as a member, includes Butte County Behavioral Health, Colusa County Behavioral Health, and Glenn County Behavioral Health and Health and Human Services Agency (HHSA).
