Partnership for Public Health (PPH) proposes a Community Paramedicine & Health Worker program in Laconia, NH, to: Increase preventative care access for underserved populations (older adults, homeless individuals). Improve chronic disease management and reduce hospital readmissions. Project Activities: Community paramedics will provide in-home health assessments, chronic disease monitoring, and minor urgent care. Community health workers will address social determinants of health, educate patients, and connect them to resources. Collaboration with hospitals, primary care, and social services will ensure seamless care transitions. Expected Outcomes: Increased preventative care visits by paramedics and health workers (targets: 100 in year 1, 200 in year 2, 300 in year 3). Improved chronic disease management (targets: 75 unique patients in year 1, 150 in year 2, 300 in year 3). Enhanced patient satisfaction and strengthened community partnerships.
