Delta Health Systems Implementation Program - Project Abstract Summary Project Title: Comprehensive Rural Healthcare Transformation: Integrating Electronic Health Records, Quality Analytics, Cybersecurity, and Advanced Diagnostic Capabilities Applicant: Pemiscot Memorial Health Systems, Hayti, Missouri (Rural Delta Region) Funding Requested: $351,403 (September 2025-August 2027) Purpose: Implement evidence-based improvements addressing four critical needs identified through technical assistance: obsolete IT infrastructure creating security vulnerabilities, lack of standardized quality measurement, inefficient paper-based documentation in our Long-Term Care Unit, and limited diagnostic capabilities for rural patients with chronic liver disease. Target Population: Rural residents of Pemiscot County, Missouri—a medically underserved Delta Region community with high poverty rates, transportation barriers, and elevated chronic disease prevalence, including liver disease due to high rates of obesity, diabetes, and alcohol use. As a Disproportionate Share Hospital, we serve a significantly higher percentage of Medicaid, uninsured, and low-income patients than other hospitals in the region. Our health system serves as the primary healthcare provider, with nearest tertiary care centers over 85 miles away. Proposed Implementation Activities: 1. PointClickCare EHR System: Implementation of specialized long-term care EHR with clinical documentation, revenue cycle management, and care planning tools. 2. HEDIS Analytics: Deployment of standardized quality measures with dashboards tracking care gaps and provider performance. 3. Cybersecurity Infrastructure Modernization: Replacement of obsolete, unsupported IT systems with Enterprise Fortress Gateways, secure switches, encrypted access points, and HIPAA-compliant Healow Telehealth platform for secure virtual care delivery. 4. Advanced Diagnostic Capabilities: Implementation of Aplio A550 ultrasound system with specialized liver analysis package to enhance early diagnosis and monitoring of liver disease in this high-risk rural population. Expected Outcomes: • 100% replacement of obsolete network equipment with supported, secure alternatives • Improvement in key HEDIS measures by 15-20% • Complete transition of LTCU patient records to electronic system • 25% reduction in documentation time and medication errors • Zero ransomware or data breach incidents • Full HIPAA Security Rule compliance • 30% increase in early detection of liver disease with advanced ultrasound capabilities • Secure virtual care delivery to at least 150 rural patients per year Methodology: Phased implementation beginning with cybersecurity infrastructure as the foundation, followed by staff training, system deployment, advanced diagnostic equipment implementation, and continuous quality improvement processes. Relevance to Program Purpose: This implementation directly aligns with the Delta Health Systems Implementation Program by improving financial sustainability through better documentation, secure operations, enhanced diagnostic capabilities, and reduced penalties while increasing access to high-quality care through standardized quality measurement, protected health information systems, and advanced diagnostic tools in this rural Delta region community. For Disproportionate Share Hospitals like ours, implementing these evidence-based interventions will create particularly significant value by improving quality metrics that affect DSH funding calculations while enhancing care for our high-risk population.
