Hannahville's Elder Fall Prevention Project - The Hannahville Indian Community is a federally recognized Indian Tribe residing in Michigan’s Upper Peninsula in a very rural area. Currently, there are a total of 192 enrolled Elders with 138 either living on the reservation or within the service area. The Hannahville Department of Health and Human Services (Health Center) is located on the reservation and mainly serves tribal members and their families, as well as other Native Americans that live on the reservation or within our local area. Within the Health Center, the Community Health Department provides direct services to Elders through public health programming and in-home services. In applying for TIPCAP Part II funding, the Community Health staff anticipates being able to expand the number of services offered to Elders by achieving the following goals: 1) train staff to conduct fall assessments, 2) train staff to properly detect safety hazards in homes, and 3) offer appropriate safety equipment in a timely manner. As we have data from a Hannahville Elder Needs Assessment that was completed in June and July of 2025, our expected long-term outcomes for Elders after completing fall assessments and providing education would be to decrease 1) the amount of falls, 2) the injury rates from falls, and 3) the amount of Elders who worry about falls, while indirectly increasing the rate of Elders who 1) participate in the In-Home Services Program and 2) follow-through with referrals for outside medical care.
