Pine Hill Health Center Coordinated Care Management Systems Improvement Project - Ramah Navajo School Board, Inc. (RNSB) requests $300,000 for a 36-month Health Management Structure project to strengthen the management systems supporting Pine Hill Health Center (PHHC) in Pine Hill, New Mexico. PHHC serves the Ramah Navajo community, including an IHS user population of 2,680 reported in 2024. PHHC has experienced growth in services, staffing, administrative responsibilities, and coordinated-care needs. Review of current policies, procedures, operational records, staffing information, and manager-identified deficiencies shows that important functions are not consistently governed as one current, documented, and measurable system. Priority weaknesses include outdated or incomplete policies; fragmented departmental workflows; manual coding and billing processes; inconsistent procurement, inventory, and accountability controls; gaps in medical-record and referral handoffs; incomplete orientation, training, credentialing, and privileging processes; manual quality monitoring; and limited information-system and downtime resilience. Over the project period, RNSB will establish a controlled policy lifecycle; map and redesign priority workflows; strengthen internal controls; clarify roles and responsibilities; develop workforce, credentialing, privileging, coordinated-care, and training frameworks; conduct a vendor-neutral technology requirements and alternatives analysis; implement approved improvements; and establish performance dashboards, corrective-action monitoring, and sustainability procedures. Technology decisions will protect required IHS interfaces, reporting, privacy, cybersecurity, continuity, and service delivery. The project will produce current and accessible policies, coordinated workflows, stronger fiscal and operational controls, trained personnel, defined technology requirements, measurable performance information, and a sustainable management-system framework. These improvements will strengthen PHHC's capacity to manage its Title I health programs, support coordinated care, protect patient services, improve accountability, and maintain continuous improvement after grant funding ends.
