Tribal Management Grant- Health Management Structure - Indian Health Council, Inc., a consortium of nine federally recognized tribes located in north San Diego County, will apply the 3-year, $300,000 funding from the Indian Health Services Tribal Management Grant Program, project type 4 - Health Management Structure with multi-layered organizational focus, planning, development and phased integration at the executive team level in Year 1, full program implementation in Year 2 and active engagement and utilization by all IHC team members and community in Year 3. The project goal is to establish consistent commitment and ownership of the IHC Mission and Vision to ensure the greatest health management outcomes possible. The accomplish this, IHC will complete the following outcomes and project deliverables: (1) Strategic Planning with emphasis on IHC’s Integrated Care Model, (2) Community-Driven Communication Program, and (3) Health Information Modernization and Training. By enhancing these key areas, IHC will build upon previous outcomes and enhance overall tribal health management infrastructure and patient service delivery. Project Objectives for Year 1 are as follows: Objective 1: By the end of Year 1, the executive team will establish measurable Organization-wide Strategic Goals with emphasis on IHC’s Integrated Care Model. Objective 2: By the end of Year 1, the executive team will debut the updated community-driven communication plan to drive overall organizational and community success. Objective 3: By the end of Year 1, the EHR Systems Analyst will train and fully integrate the Health Information Modernization tools to ensure minimal errors, reduced workflow challenges and maximized billing efficiency. The Executive Team will develop mechanisms to drive shared commitment to the IHC Mission, Vision, Strategic Goals and the unique roles and shared responsibilities that every employee and patient plays in achieving overall health and wellness.
