The Children’s Home Society of New Jersey (CHSofNJ) Perinatal Community Integration Model (PCIM), will operate in Mercer, Monmouth, and Ocean Counties, New Jersey, and the US Territory of Puerto Rico. Within these geographies, CHSofNJ will focus on municipalities and areas where the needs are observed to be the greatest in relation to perinatal health outcomes. These populations tend to be living in poverty and are at increased risk of poor maternal and infant health outcomes and social determinants of health, which will be measured, monitored and evaluated throughout the grant period.
The three components of PCIM will sustain and expand the CHSofNJ maternal child health continuum and support expansion in the US Territory of Puerto Rico - (1) case management and referral services; (2) planned integration with perinatal medical provider systems in Mercer County and enhanced hospital integration in Ocean and Monmouth County and Puerto Rico; and (3) community health worker and doula support. These activities are aimed to improve social determinants of health, increase access to prenatal care, and reduce maternal and child health outcome disparities, particularly those identified by Healthy People 2030 measures.
CHSofNJ will replicate the innovative PCIM based on 20 years of successful community-based program operation and strong, positive outcomes in Mercer County. The agency will partner with community based non-profits, perinatal medical providers, and mental health providers to create safe and culturally responsive services for pregnant and newly parenting women.
