In fiscal year 2022-2023 DIHFS Family Centered Approaches to Preventing Type 2 Diabetes project will continue to focus on the Denver Metro jurisdiction counties and the AI/AN population. We plan to serve a total of 60 unduplicated participants through direct intervention programming of Shared Medical Appointments, Healthy Lifestyle Coach 1:1 coaching, Registered Dietitian 1:1 nutrition counseling, healthy lifestyle workshops, and community resource referrals.
