Clayton County Health District proposes to implement the Community Health Literacy Initiative (CHLI). In partnership with Fulton and DeKalb Counties, CHLI will provide health literacy prevention and intervention strategies in the Southern Crescent with one of the highest Social Vulnerability Indexes and COVID ? 19 positivity rates in the state. The Southern Crescent is comprised of Clayton County and the southern portions of Fulton and DeKalb Counties and South Atlanta. The area of the Southern Crescent is 89% ethnic minority, 82% Black and 6% Hispanic/Latino. Other partners include Morehouse School of Medicine, Southside Medical Center FQHCs, Clayton County Library and several other community and faith-based organizations. This collaborative will develop and implement a Health Literacy Toolkit for the residents of the Southern Crescent. The Toolkit provides 1) Health Literacy for Communities, 2) Health Literacy for Medical Professionals and Public Officials, and 3) Community Health Outreach Worker Training for Sustainability. The goals are to 1. Increase health literacy related to COVID in the target area, and 2. Increase provider knowledge of patient health literacy needs and evidence-based (EB) strategies to improve provider-patient communication, patient understanding, decision-making and follow-through. An independent evaluation team will lead a CQI and outcome evaluation to determine effectiveness of the program and strategies.
