The Partnerships in Health Project, provided by the Council on Substance Abuse (COSA)-NCADD in Montgomery, Autauga, Elmore and Lowndes counties in central Alabama. There are many African American adults with both substance use disorders and diabetes/pre-diabetes. It is believed that integrating Peer Recovery Support Services with Diabetes Self-Management Education and supplemented by physical activity, will provide a more holistic recovery program for these persons in our region. The project has successfully provided services to many persons and will continue to provide services while developing a sustainability plan for future services. The goals of the project are: 1: To sustain and provide for expansion of the Partnerships in Health project which integrates substance use recovery with diabetes education and intervention in Montgomery, Autauga, Elmore and Lowndes Counties of Alabama. 1.1 Develop a comprehensive sustainability plan with project partners to include plans for finances, staffing/supervision, and material resources. 1.2 Implement the sustainability plan with project partners. 1.3 Continue and expand the current Partnerships in Health project throughout the year while developing sustainability.
