Address: 19990 Governors Highway; Olympia Fields, IL 60461 Project Director: Audrey Pennington, Chief Operations Officer Phone: 773-802-7645; Fax: 708-747-0170 Email: aapennington@auntmarthas.org Website Address: www.auntmarthas.org Grant Funds requested: $450,000 PROJECT ABSTRACT Brief description of the proposed project: Funding from the HRSA Community Project Funding / Congressionally Directed Spending (CPF/CDS) will support the modernization and implementation of a mental and behavioral health-focused Integrated Practice Unit (IPU) at Aunt Martha’s Kankakee Health Center located at 1777 E. Court Street, in Kankakee, IL—located in Kankakee County. Redesigning and renovating a substantial portion of existing facility to accommodate the IPU model; purchasing fixed equipment essential for delivering quality care to achieve the goals of improving health outcomes, increasing patient satisfaction, and reducing health care costs for a vulnerable population in Kankakee County. With the opportunity to access 2024 Congressionally-Directed Spending, Aunt Martha’s is prepared to formally expand our implementation of our value-based IPU model at our clinic in Kankakee, IL. An IPU embodies the central principle of a value-driven organization: to organize around customer needs, not the supply of particular services. IPUs represent a profound change in health care delivery, enabling better, more efficient care for groups of patients with similar needs: using multidisciplinary teams, ideally co-located, that are structured to meet the needs of well-de?ned groups of patients, usually with similar conditions, over the full cycle of care. A critical function of a primary care IPU is to identify patients who will benefit from our internal specialty services (e.g., behavioral health, Family Planning) as well as from specialty IPUs external to the practice, and serve as the central point of coordination and collaboration with those teams and among our partners in Kankakee County. Needs to be addressed: The project will focus on adolescent health needs, including integrated primary care and behavioral health services. Currently, adolescents and teenagers in northeastern Illinois and across the country are experiencing elevated rates of mental health challenges. According to survey data reported by the 2020 IL Youth Survey for Cook County, 32% of teens experienced depression, and 14% have considered suicide. Proposed Services: Aunt Martha’s will use an IPU model that offers value-based, integrated primary care and behavioral health services in a leading-edge patient-focused, trauma-informed care model. Population to be served: This expansion would enable the facility to serve more than 3,000 children and youth ages 6-18 who are patients of our Kankakee Health Center.
