ANHC services include primary medical, dental, behavioral health counseling, lab and imaging services, pharmacy, and health education and promotion activities for all stages of life. Population Served: ANHC’s target population is low-income, uninsured, and underinsured individuals, including people covered by Medicaid and Medicare, and people experiencing homelessness. In 2022, ANHC served 11,397 unduplicated patients; 856 were homeless. The majority of ANHC’s patients are low-income – of those with known incomes in 2022, fewer than 10% of patients had household incomes above 200% of the federal poverty level (FPL). In 2022, nearly 16% of all ANHC patients were uninsured and another 35% were covered by Medicaid, and another 22% were covered by Medicare. Proposed Project: ANHC provides a full range of family practice primary care services in a patient-centered medical home, including prevention, diagnosis, and treatment of acute and chronic illness; chronic disease management; and referrals and follow-up for specialty care. ANHC continues to evaluate additional approaches and methods to best serve our patients and community, including continued maintenance and care for the state-of-the art facility patients are seen and served in. Through the support of the Community Project Funding / Congressionally Directed Funding (FY-2023) Facilities Project funding, ANHC proposes the purchase and installation of supplies and equipment to serve two projects: (1) Replacement of the Rooftop Units responsible for building air circulation; and (2) In-building Lighting retrofit. The proposed project seeks to replace aging equipment with up-to-date elements that provide an increase in operational efficiencies and efficacy, while decreasing the less favorable financial and environmental impacts of older equipment. (1) The replacement of the Rooftop Units responsible for building air circulation will facilitate the purchase of two fixed equipment pieces, which will replace the existing building rooftop unit, using the existing rooftop space, connections, and wiring for installation. (2) The in-building lighting retrofit will replace all current interior lighting fixtures (fluorescent bulb fixtures) in the facility with LED lighting fixtures able to be controlled through both dimmable wall switches, as well as through motion sensors, ensuring lights are off when a space is not in use. The replacement light fixtures are 'drop-in' installation, meaning that no new modifications to the facility are needed for their installation, as they are able to utilize the existing locations of the current fluorescent lighting fixtures. ANHC intends to use this facility modernization with fixed equipment to decrease the organization’s overall carbon footprint by utilizing energy efficient models to replace the aging assets. Additionally, this will reduce the overall costs to the organization’s facility operating budget, enabling reinvestment of saved resources back into patient-centered care and support.
