Congressionally Directed Spending for Construction Projects - Bay Area Community Health (BACH) is a Federally Qualified Health Center (FQHC) providing comprehensive health services in Alameda and Santa Clara Counties, California. BACH was formed in 2020 off the strength of southern Alameda County’s Tri-City Health Center and Santa Clara County’s Foothill Community Health Center. Formed during the middle of the COVID-19 pandemic, BACH brings more than 70 years of combined service to the service area. Today, BACH serves nearly 70,000 people who rely on high-quality healthcare services, regardless of their immigration status, race, ethnicity, disability, gender identity, insurance status, or ability to pay. Through the years, BACH has expanded access to care to individuals and families living in Alameda and Santa Clara County who are uninsured or underinsured by increasing operating hours to include evening and weekend hours. The demand for affordable primary and preventive care within the two counties continues to grow, with the disparities between the wealthy and the poor becoming increasingly dramatic. Fremont, California is a city located in Alameda County, in the San Francisco Bay Area. Fremont has been recognized as a city with a diverse population, including individuals from various ethnic backgrounds and countries of origin. The city has been a hub for immigrant communities, including Afghan residents and their families. Afghans have been settling in Fremont for several decades, particularly after political instability and conflicts in Afghanistan led to waves of migration. Many Afghan immigrants and refugees have chosen to make Fremont their home due to factors such as job opportunities, educational resources, and the presence of existing Afghan communities. Using Community Project Funding/Congressional Directed Spending, BACH will work to address this demand by purchasing two mobile clinics to service the afghan refugee population within the Centerville District of Fremont. The new delivery mobile sites will offer culturally appropriate, comprehensive, primary and preventive health care, behavioral health and other ancillary services using a walk-in, same day and appointment schedule for those who need alternate clinic hours, including evenings and Saturday availability. A Patient Centered Medical Home model will be employed to ensure patients with chronic conditions have ongoing access to care and support services.
