As an FQHC, Mid-State operates with a clear and collective vision of providing high-quality primary care services to its geographically isolated, rural community and is committed to the delivery of a coordinated, personalized care experience as part of an innovative community health model. Mid-State fosters strong collaborative relationships with a wide variety of public and private entities within its service area and the State to support access to health care services in its effort to promote a healthier community and population. Service Area and Target Population Needs: Mid-State serves over 12,000 of the 31,788 residents of the eighteen rural communities in central New Hampshire that are designated as Medically Underserved Areas (MUA). Residents experience higher rates of socio-economic challenges; 30% of the region’s residents are low-income and 11% are living in poverty. Thirty-three percent of households in the region are reported as single parents with children. These challenges trickle down to the 5,000 children under the age of 18. Census estimates indicate that 12.6% of these children are living in poverty with household incomes less than 100% of the federal poverty level (FPL), and 40.8% are living with household incomes less than 200% of FPL, those rates are significantly higher than the state rates of 10.3% and 26.5%, respectively. Additionally, 5.4% of the region’s children under 5 are uninsured. These factors coupled with low rates of childhood lead testing, child wellness exams, and immunization indicate the need for a higher level of intervention to promote wellness for disadvantaged women, families, and their children. Project Summary & Services Provided: Mid-State is committed to reducing disparities in access and health status for children and their parents. To support this special population, it plans an infrastructure improvement project to renovate 3,000 sf of leased space adjacent to its Boulder Point site. The goal of the renovation is to create a space for family health and wellness where all the families’ health needs are addressed. Services will include primary medical care for infants, children, adolescents, and their parents including pre- and post-natal primary care for mothers. The integrated model will also include child and adult mental health, oral health, lactation support, parent education, and enabling services in a specially designed space to support their complex needs. The innovative design fosters a single location for health care for families with or expecting children. Features include larger exam rooms, an oral health suite equipped for all ages from infants – adults, and group meeting space to offer parent education sessions. We envision a family wellness center that is designed to support a true family medicine model where mothers/parents and their children are able to receive care in one place, and if appropriate, at the same time. The goal is to help address the challenges so many mothers/parents face while caring for themselves and their children.
