Address: 19990 Governors Highway; Olympia Fields, IL 60461 Project Director: Audrey Pennington, Chief Operations Officer Phone: 773-802-7645; Fax: 708-747-0710 Email: aapennington@auntmarthas.org Website Address: www.auntmarthas.org Grant Funds requested: $450,000 PROJECT ABSTRACT Brief description of the proposed project: Funding from the HRSA Community Project Funding / Congressionally Directed Spending (CPF/CDS) will support the modernization and implementation of an Integrated Practice Unit (IPU) at Aunt Martha’s Woodstock Community Health Center, leased from Northwestern Medicine Woodstock Hospital and expand access to high-quality mental and behavioral health care. The modernization will create six new exam rooms, two Behavioral Health offices, a care coordination office, and a space for meeting with partners to collaborate on addressing social determinants. With the opportunity of 2023 Congressionally-Directed Spending, Aunt Martha’s is prepared to formally expand our implementation of our value-based IPU model at our clinic in Woodstock, IL. An IPU embodies the central principle of a value-driven organization: to organize around customer needs, not the supply of particular services. IPUs represent a profound change in health care delivery, enabling better, more e?cient care for groups of patients with similar needs. IPUs utilize multidisciplinary teams, ideally co-located, that are structured to meet the needs of well- de?ned groups of patients, usually with similar conditions, over the full cycle of care. A critical function of a primary care IPU is to identify patients who will bene?t from our internal specialty services (e.g., behavioral health, Family Planning) as well as from specialty IPUs external to the practice, and serve as the central point of coordination and collaboration with those teams. and among our partners in McHenry County. Needs to be addressed: The project will focus on the needs of adolescent health, including integrated primary care and behavioral health services. Currently, adolescents and teenagers in northern Illinois and across the country are experiencing mental health challenges at elevated levels. According to survey data shared by the McHenry County Mental Health Board, more than four in ten high school students reported feeling persistently sad or hopeless during the pandemic. We will also look proactively through the lens of our Family Planning services to stress the importance of sexual and reproductive health of teens and young adults. Proposed Services: Aunt Martha’s will use an IPU model to offer the following population-specific services: Integrated Primary and Behavioral Health Care, including suicide and violence prevention; Family Planning services (Title X); Life skills development and transition to adulthood coaching; Chronic condition education; Substance use disorder and smoking cessation education; Reproductive Health education, including teen pregnancies; Sexually transmitted infections education, including human immunodeficiency virus (HIV). Population to be served: This expansion would enable the facility to serve more than 3,700 youth ages 10-21 in McHenry County and the surrounding area.
