Congressionally Directed Spending for Construction Projects - Funding Opportunity Number: HRSA-23-117 CFDA(s) Applicant Name: Marillac Clinic, Inc. DBA MarillacHealth Descriptive Title of Applicant's Project: Marillac Clinic Capital Expansion Need to Be Addressed MarillacHealth, a western Colorado non-profit and federally qualified community health center (FQHC), provides primary medical, dental and behavioral health care for 10-12,000 low-income persons residing in Mesa County (Colorado’s fourth largest county). By constructing and opening a new healthcare clinic in 2025, Marillac will expand access to health care for lower income families throughout the east end of the Grand Valley, which includes the towns of Fruitvale, Clifton and Palisade (FCP). Clifton and the surrounding area are now distinguished as a healthcare desert; despite its location in the very center of Mesa County and its densely populated lower-to-middle income neighborhoods. There is no primary medical, dental or behavioral health care available in this community. The entire FCP area is home to over 50,182 residents and includes more than 1,000 families with a single-female head of household. The median annual household income for FCP is $42,247, compared to Mesa County’s average of $55,379 and Colorado’s $62,843. Throughout FCP, Medicaid enrollment is 31%. Clifton’s Medicaid rate of 38.4% ranks as the highest in Mesa County. About 10% of FCP residents lack health uninsurance. Proposed Services Serving Mesa County for 35 years, MarillacHealth is a mission-based healthcare organization. Marillac care teams take pride in earning trust and respectfully serving complex patients whose health may be affected by tobacco, alcohol, opioids, poor nutrition and self-care. Marillac’s integrated care allows each patient the opportunity to receive abbreviated behavioral health care during any medical visit. As needed, Marillac’s wraparound care also addresses social determinants of health such as housing, transportation and food security. Marillac’s plan to establish a new healthcare center is called “New Doors to Health” because it will expand medical, dental, behavioral and new pharmacy healthcare services to 4,500 new patients. A talented pediatric team will serve the medical, dental and behavioral health needs of children, 0-17. The new 23,849 SF health center will be built from the ground-up at 512 29 ½ Road, Grand Junction, CO 81504-5383. It will occupy 4.53 acres, create 18-22 new jobs over time and include a pharmacy with indoor counter service and drive-through access. Ground-breaking will occur in early 2024; construction will be completed in 11-12 months; the grand opening will occur in the first quarter of 2025. Population Groups to Be Served Persons of all ages, birth to end of life, are welcomed at MarillacHealth. As an FQHC, our target population includes vulnerable groups such as uninsured and underinsured persons, migrant workers and their families, undocumented residents, persons impacted by homelessness and unemployment, persons on Medicaid, and everyone with annual incomes of 200% of the Federal Poverty Guideline or less. Marillac’s Sliding Fee Scale enables patients to qualify for health care services that are substantially discounted, based on the number of persons in their household and their verified annual income. About half of Marillac patients are enrolled in Colorado’s Medicaid/CHP+ public insurance programs. Marillac serves patients of all ages, ethnicities, cultures, sexual orientations, genders, religions, languages and citizenship status. All will find a welcoming healthcare home at Marillac, where our message is, we’re “committed to a healthier you.”
