This project will substantially renovate 7,000 square feet within the existing 10,007 square foot first floor of the 12,867 square foot, two story Beacon Health Center. The interior renovation will include a new 6 exam room Urgent Care suite, 36 seat waiting room, and 10 intake workstations with mechanical and electrical system upgrades. New furniture, workstations, and medical and I.T. equipment will be provided throughout the renovated area. In addition, the project includes building façade renovations which will consist of new Main Street storefront entrance door, masonry restoration, canopy, lighting, and signage. - Needs to be addressed –. Sun River Health maintains a busy Health Center site in Beacon serving over 7,000 predominantly low-income, racially and ethnically diverse patients. The site is functioning at capacity, and at the same time, Sun River Health is seeking opportunities to restructure its service delivery to respond to the escalating inappropriate use of hospital ERs, and the need to expand capacity at the Beacon site. - Proposed services – The proposed project is an alteration and renovation of existing space within the facility’s existing footprint as well as minor modifications and repairs to the building façade. The interior renovation includes improvement and reconfiguration of this existing space, installation of permanently affixed equipment, HVAC modifications, and electrical upgrades and plumbing work. The redesign of the Beacon Health Center will enable Sun River Health to add Urgent Care services at the site and will provide a dramatically redesigned system of service delivery enabling the Health Center to expand access to the comprehensive primary, preventive, behavioral, and oral health care services available at the site. - Population group(s) to be served – The proposed project will expand and improve access to critical health care services for the more than 7,000 patients currently accessing care at the Beacon Health Center as well as residents of the surrounding community in need of a medical home. These populations are predominantly low-income, and many are Medicaid enrollees or uninsured. The patient population at the Beacon Health Center is highly diverse with significant representation from racial and ethnic minorities – predominantly Hispanics/ Latinos and non-Hispanic Blacks/African Americans – and many are best served in a language other than English. In addition, the Beacon Health Center serves unique special populations including individuals experiencing homelessness, residents of public housing, young children, medically frail elderly, new and expectant mothers, people with HIV, individuals with substance use disorder, and many others.
