Website: http://www.lampreyhealth.org/ Grant Program: Section 330 (e) Community Health Center (CHC), $3,588,298 Lamprey Health Care (Lamprey) is New Hampshire’s (NH) first community health center, formed in 1971. LHC operates three health centers in Raymond, Newmarket and Nashua, two sites co-located within a mental health centers located in Nashua – InteGreat Health, and Derry – Healthy Together, a site located at the Nashua Soup Kitchen and Shelter, and a Mobile Health Unit. LHC is recognized for its excellence and quality, receiving the Health Resources and Services Administration’s Health Center Quality Leader – Gold Award ranking in the top 10% of all health centers. LHC is also recognized by the National Committee for Quality Assurance as a Patient Centered Medical Home. Lamprey is proposing the alteration and renovation of our Nashua Center. Located in the heart of Nashua, New Hampshire’s second largest city, the Nashua Center serves over 5,600 patients providing over 22,000 visits annually. The Nashua Center is adjacent to a medically underserved population. In 2022 89% of Nashua Health Center patients earned 200% or less than Federal Poverty Guidelines (FPG), 60% are best served by a language other than English, and 63% are ethnic or racial minorities. Since occupying this facility in 2011, Lamprey has added integrated behavioral health services and substance use disorder treatment to our service offerings in Nashua but have not expanded our space to accommodate the service expansion. In addition, there is sufficient demand to warrant expanding primary care services. Lamprey’s proposed project is the alteration, repair, and renovation of our Nashua Center located at 22 Prospect Street in Nashua, NH. The overall goals of the project are to expand clinical space and reconfigure existing space for improved workflows, which will increase patient access and improve the patient’s experience. The project will allow Lamprey to increase access s by expanding clinical space in the currently unfinished second story of the 2011 facility addition as well as create a more efficient workflow for clinical staff. Proposed changes include: • Reconfiguration of the existing reception and waiting area; • Easier and confidential access for patients to financial counseling services; • Embedding behavioral health team members within each clinical team; • Creating a food pantry space for ease of patient access; • Altering space to decrease the distance between the patient service representatives and medical assistants to improve communication; • Creation of a bathroom and exam room using universal and inclusive design (based on both ADA and bariatric guidelines); • Enhanced way finding in the renovation; • Build-out of seven additional exam rooms on the second floor, point of service internal lab; three behavioral health rooms, and lactation/precepting office. To maintain access, the project will be conducted in phases. The proposed completion date for the project is spring of 2024.
