Congressionally Directed Spending for Construction Projects - In Jackson County, Missouri, for older adults living at low income levels with chronic medical conditions but preferring to age in place at home, the options to access the comprehensive healthcare they need are limited. The purpose of this project is to introduce a PACE program – Programs of All-Inclusive Care for the Elderly – to serve about 350 people at a time. PACE KC is a major, transformational new program of Swope Health which is Kansas City’s largest federally qualified health center serving over 44,000 people each year. Founded in 1969, Swope Health has grown from serving patients in a church basement to being a leading FQHC on a national level with a reputation for continuous improvement through innovation. In keeping with our history and leaning on our experience, using the national PACE model, Swope Health will build a new 33,000 square foot PACE KC Adult Wellness Center that is slated to open in 2023. This will be only the second PACE program in the state of Missouri and the first in Jackson County. The program, which is projected to enroll 350 members within its first five years, will bring over 90 new jobs to 4141 Dr. MLK Jr., Boulevard in Kansas City, Missouri, 64130. This zip code is the most economically distressed in Kansas City. The median household income is $37,679, compared to $56,96 for Jackson County, Missouri. PACE members are aged 55 and over, living at very low income levels and managing multiple and/or chronic and complicated health conditions, such as diabetes, dementia or chronic heart disease. PACE is a day center-based model delivered at no cost to members and with many empirically proven outcomes such as: PACE members are healthier and ultimately live longer than their counterparts in nursing home care, hospitalizations are reduced, and families have a 97.5% satisfaction level. The Medicare/Medicaid cost to provide PACE is 15% lower than the cost of institutional based care. PACE KC will offer primary care, dental, behavioral health, vision, lab, pharmacy, rehabilitation, transportation and other social services to prevent isolation. The quality, comprehensive healthcare will be life changing for our PACE KC members suffering from complicated health conditions.The PACE KC team will seek to understand the trials that face our members, continuing our long history of addressing health inequities by breaking through the barriers that prevent people from receiving the healthcare they need. From cost to appointment access to transportation, we will remove these inequitable barriers for our members. Swope will leverage national PACE program benchmark data to assure we are exceeding outcomes in quality of care, reducing hospital visits, and always elevating customer satisfaction. PACE KC members will ultimately live longer with reduced burden on caregivers at home, many sandwiched between caring for two or more generations. Swope Health respectfully requests $2 million in support toward construction of the PACE KC Adult Wellness Center. These funds ultimately go straight back into the community in terms of economic development, job creation and payroll, alongside the launch of a much-needed new care model for older adults in Jackson County.
