The overall project renovates the existing two-story masonry 23,224 square foot (sf) facility to provide improved access through ground floor prompt care expansion (4,930 sf.), improved space for hosted and Sun River behavioral health and substance use care, general infrastructure improvements, creation of community services and meeting space as well as improvements to finishes, and mechanical infrastructure for the 5,600 sf co-located Alpha School. A key aspect of the project is providing the site with an elevator (currently not in the existing facility) to allow second floor services and spaces to become accessible. The proposed $1.8 million CPF/CDS project funding will be apportioned across the project including: second floor community room renovations with rooftop mechanical and electrical system upgrades. New furniture, workstations, and allowable equipment will be provided throughout the first and second floors. The exterior improvements will include front plaza concrete paving, planter beds, landscaping, and fences. The building façade renovations will consist of masonry restoration/painting, a canopy, lighting, and signage. - Needs to be addressed –. The proposed project seeks to address the need for expanded access to primary care and behavioral health services in the East New York neighborhood of Brooklyn in New York City. By creating a ‘one-stop’ service delivery model where primary care and behavioral health can be accessed through a single location, the project will address the vast health disparities and high rates of avoidable hospitalization use in the service area. - Proposed services – The proposed project is an alteration and renovation of existing space within the facility’s existing footprint as well as minor modifications, repairs, and improvements to the building’s exterior. The interior renovation includes improvement and reconfiguration of this existing space, installation of permanently affixed equipment, HVAC modifications, and electrical upgrades and plumbing work. The redesign of the Alpha School Health Center will enable Sun River Health to expand access to primary care, behavioral health, and mental health care at the site. - Population group(s) to be served – The proposed project will expand and improve access to critical health care services for the patients currently accessing care at the Alpha School Health Center as well as residents of the surrounding community in need of a medical home. These populations are predominantly low-income, and many are Medicaid enrollees or uninsured. The patient population at the Alpha School Health Center is highly diverse with significant representation from racial and ethnic minorities – predominantly Hispanics/ Latinos and non-Hispanic Blacks/African Americans – and many are best served in a language other than English. In addition, the Alpha School Health Center serves unique special populations including individuals experiencing homelessness, residents of public housing, at-risk youth, people with HIV, those with serious and persistent mental illness, individuals with substance use disorder, and many others.
