Congressionally Directed Spending for Construction Projects - Southwest is establishing a full-service site on the second floor of the new 43,000 square foot Wakeman Boys and Girls Club facility on Madison Avenue in Bridgeport. We will provide accessible primary care services, including medical, dental, and behavioral health care for patients of all ages, regardless of their ability to pay. Patients served at this location will have access to the services available at other Southwest locations, such as orthopedics, chiropractic, acupuncture, and podiatry. Southwest will also provide supportive services, such as arranging for transportation, referrals for care beyond the scope of primary care, and make linkages with other service agencies in the community that provide additional resources like food, heating fuel, rent assistance among others. The full scope of the project has been divided into 3 phases and will occupy 8,467 square feet includes 11 medical exam rooms, including 1 procedure room and 1 isolation room, 4 dental operatories, 4 behavioral health treatment rooms, and a lab. The funding from this project will support the first phase and encompass 8 exam rooms, 2 behavioral health treatment rooms, and a lab and total approximately half of the total square footage. The collaboration between Wakeman and Southwest represents a unique model that will weave together complimentary services to offer a broader spectrum of resources to the Wakeman members, Southwest patients, their families, and the community surrounding the new Clubhouse. Healthcare services will be delivered in a family practice model so patients of all ages can come to this site to address their healthcare needs. Specific programs, such as Cognitive Behavior Intervention for Trauma, will be offered by Southwest to support Wakeman members. Wakeman will provide childcare for parents so they can seek their care without juggling children. Additionally, Wakeman is participating in the USDA Child and Adult Care Food Program. Southwest will provide nutrition services for members and patients alike and refer patients to this resource for healthy, nutritious meals that help all patients, particularly those with chronic diseases such as diabetes and hypertension, achieve and maintain good health. These are just a few examples of the way partnership will enhance the services provided by both organizations, offering more comprehensive, wholistic care and support.
