Project Director: Suzanne Lagarde MD MBA Contact Numbers: 203-314-8134 (voice); 203-777-8506 (fax) E-mail: s.lagarde@fhchc.org Web: www.fhchc.org Funding: $3,000,000 in Congressionally Directed Spending funds Background: Founded in 1971, Fair Haven Community Health Clinic, inc (FHCHC) currently provides comprehensive primary care services to over 32,000 residents of the greater New Haven CT area. In 2022, FHCHC served a population that is 91% racial or ethnic minority (61% self-report as Latino and 45% as African American). Ninety-three percent of patients live at or below 200% of Federal Poverty Levels. Since the early 1980s, the primary site of service has been 374 Grand Avenue, in the Fair Haven neighborhood of New Haven. At this location, care is provided in three interconnected Victorian houses that were built in the 1800s. The site is no longer large enough nor well-suited to provide state-of-the-art primary care to the community. Proposal: In its first major capital project since its founding over 50 years ago, FHCHC proposes to build a new, three story, 35,560 square foot facility on property immediately adjacent to its current clinic at 374 Grand Avenue. This property was purchased by FHCHC in 2020. Services to be provided at this new location will include adult and pediatric medical services, as well as BH and SUD care. Parking for the new facility will be supported through an upgrade and expansion of our current parking lot. This requires the demolition of three small adjacent buildings all currently owned by FHCHC. The new building will add 26 exam rooms, 2 negative pressure rooms, a laboratory and a pharmacy. BH rooms will allow for integration with medical services. In response to input by community, there will be two large meeting/training rooms which will be available for staff trainings as well as for community programs such as digital literacy training, ESL classes and after school programs. Construction is currently scheduled for completion in the first half of 2025. Prevailing wages will be paid during this project. The construction will be overseen by a construction manager with a goal of awarding a majority of subcontracts to a combination of minority-owned, women-owned and small businesses. Projected Outcomes: This project is estimated to yield 50 FTE construction jobs. When completed, the new site will yield an additional 41 jobs, consisting of medical staff, patient support staff and administrative staff. The new construction site will become a part of the FHCHC Fair Haven campus, ie the new building will supplement, not replace our current clinic at 374 Grand Avenue. By the end of CY 2027, an additional 3000 unique individuals will be served.
