This application is to seek $1,000,000 of funding to expand high-quality integrated health services in Warwick, RI. Our existing center, the Everett C. Wilcox Health Center, has 6 primary care exam rooms, 3 dental operatories and 3 behavioral health offices but we have outgrown our space. To mitigate this need, we have purchased a new building site next door to renovate/rebuild and will add three exam rooms, a laboratory, a procedure room and a second floor with elevator for offices. This will total 10,000 square feet for primary care services. Reflecting both the vision and mission of CCAP, increasing integrated health care access benefits low-income residents of Warwick and the surrounding communities. With over 15,000 integrated health center patients and a large wait list for dental and behavioral health services, there is a need for increased access. In addition, it is challenging for low-income residents to find medical homes willing to accept new patients and their insurance if insured at all. CCAP is the FQHC for uninsured, underinsured and publicly insured residents in Warwick, RI. In addition to integrated health services, this Warwick site supports maternal child home visiting services (MIECHV funded), the Warwick Health Equity Zone, and the Warwick Substance Abuse Task Force. Once the new health facility is built on the same 'campus', we will expand the current Wilcox building for dental services and add five operatories. As stated, our main reason for purchasing the additional building in Warwick is to increase primary care, specifically adding primary care exam rooms, a procedure room, a laboratory, and administrative office for staff on a second floor (with an elevator for ADA compliance/further expansion). The new 5,000 square foot building purchased, on one floor, had housed a beauty salon and martial arts center in the past and was now vacant. It is located within the small shopping center that currently houses our Wilcox Health Center; the location is perfect for expansion. In the past two years, our healthcare enrollment has increased from slightly over 10,000 unduplicated patients to over 15,000 at all locations. Current primary care patients at the Wilcox Center alone include approximately 3,000 patients. With the expansion of additional exam rooms and other services at the adjoining site, we can significantly increase access and ensure primary care for all individuals who request services. The new health center will be 10,000 square feet when completed and when fully operational, primary care will shift to the new building, and we will seek to expand our dental program at the existing site.
