Valley Family is constructing a 10,075 sq. ft. Integrated Medical-Dental-Behavioral Health Clinic in Nyssa, OR to improve access to care and increase the number of rural patients served. - Valley Family Health Care will construct a 10,075 sq.ft. “integrated” clinic in Nyssa, Oregon that incorporates behavioral health services, medical, and dental care, along with nurse case management, community health workers, insurance enrollment, and other supportive services. The new clinic will feature on-site behavioral health clinicians to treat patients, in addition to behavioral health navigators to help patients obtain mental health screenings, counseling, and referrals to community-based supports and services. All services will be offered on a sliding scale, with no one turned away for lack of ability to pay. The primary benefit to the community will be the replacement of the aging physical facilities that currently house our dental and medical clinics in Nyssa, with a new combined clinic that is specifically designed to facilitate integration of care for our patients. The physical layout of the facility will encourage collaboration among and between medical/dental/behavioral health teams. We expect the new facility to more readily attract new patients, and to be more appealing to staff and community visitors. Access to care will be improved by reducing travel barriers and minimizing missed work time. Our existing dental and medical clinics (one each) in Nyssa served just over 2,700 residents in Calendar 2025 with 6,869 medical (and behavioral health) visits and 2,690 dental visits. We project that number to increase to 3,200 residents after the first full year of integrated clinic operations. The projected increase is based on improved service capacity, co-location of services, and the ability to recruit additional providers into a modern facility. Our existing Dental clinic in Nyssa was built 87 years ago, and the Medical clinic 64 years ago. The physical size and design of each of these buildings limit our ability to provide integrated care, while the structures do not lend themselves to feasible expansion. Importantly, we also project that a higher percentage of these patients will avail themselves of “combined” services (medical and/or dental and/or behavioral or other services) than patients in our other clinics. Currently, only 10% of our patients participate in more than one service line. We are currently negotiating with the City of Nyssa and with another health organization that holds property in Nyssa for either a land purchase or a land exchange option to serve as the new clinic construction site. As detailed in our project budget, we are applying for regional foundation grants, requesting financial support from the Oregon state legislature, and have capital reserves committed to this construction project. We plan to sell our current Nyssa Dental Clinic to help finance the new facility, and may utilize the Nyssa Medical Clinic in a property exchange with the City of Nyssa. Our budget narrative includes $625,000 line item for land acquisition. This is the anticipated value of the exchanged property or the cash price for a direct acquisition. Preliminary construction documents have been prepared, and are attached to this application. Letters of support have been obtained from the District 14 office of the Oregon Department of Human Services, the Malheur County Court, and the City of Nyssa. We are requesting a construction-no equipment grant. We anticipate the project will commence within 30 days of award notification, and we estimate the construction will be completed by August of 2028, with the new clinic fully operational before the end of 2028.
