Kings Canyon Road, Fresno, CA 93727. The Fresno Center ADHC is an Adult Day Health Care (ADHC)/Community-Based Adult Services (CBAS) center/program that provides culturally and linguistically competent health and recovery support services to Fresno County’s culturally and ethnically diverse adult population (age 18-101), including immigrant and refugee, with chronic medical, cognitive, or mental health conditions and/or disabilities that are at risk of needing institutional care. The project is a moveable equipment only project. Through this project, The Fresno Center ADHC will purchase moveable equipment (including HIPPA compliant, mental/behavioral health client management, Electronic Health Record (EHR) software/system; a minibus; and mobile vans, including those that are commercial handicap wheelchair accessible and ADA compliant) that will enhance service delivery and address any transportation barriers that clients may be experiencing. This project will ensure the hard-to-serve, unserved and/or inappropriately served cultural populations have increased access to culturally and linguistically competent mental health and recovery services that promote prevention or treatment of mental health disorders, including rehabilitation, outreach and other support services, and substance abuse.
