Community Project Funding/Congressionally Directed Spending - Construction - Pediatric & Family Medical Center dba Eisner Health respectfully requests funding to support an Equipment Only project aimed at increasing access to care to residents of Lynwood, California and the surrounding community at our Lynwood, California health center site, located at 3680 E. Imperial Highway #200, Lynwood, CA 90262. Eisner Health currently occupies the space under a lease agreement. All equipment purchased through this request will be stored at the aforementioned Lynwood health center site. In 2021, we served 4,293 unduplicated patients at our Lynwood Health Center site and provided care for 2,871 patients residing in the five Zip Codes that make up the greater Lynwood area (90002, 90059, 90221, 90262, and 90280). Current services at Eisner Health Lynwood include pediatric and adult medical care, prenatal care, women’s health, pediatric and adult dental, case management, behavioral health via telehealth and health education classes.
