The dynamic needs of behavioral health patients require equally dynamic resource capabilities to meet them. Since 2019, Texas Children’s Hospital (TCH) has seen a significant rise in the need for mental and behavioral health resources. On average, over 400 patients present to the TCH emergency rooms each month since October 2021 for a mental and behavioral health concern. Patients can end up admitted to high cost inpatient facilities due to the lack of step down programs available to manage high acuity cases that require more care than can be provided in a traditional outpatient environment. As part of the continuum of care for patients with mental and behavioral health needs, Texas Children’s is launching an Intensive Outpatient Program (IOP). Currently, Houston and the surrounding counties comprises only 6 Intensive Outpatient Programs for children and adolescents, and only a subset of those accept patients with Medicaid insurance coverage. Texas Children’s Hospital has leased approximately 17,500 square feet shell space for build out to provide a comprehensive IOP program designed to meet patient needs. The space will comprise 12 individual treatment rooms for individual and family therapy, four large rooms for group and multifamily therapy, and three rooms dedicated for recreational therapy modalities. Clinical and administrative staff will have designated and separated office space for treatment team conferences, documentation needs, and administrative tasks. The space comprises ligature resistant materials, furniture, and build outs to ensure patient safety throughout program duration. The Texas Children’s Hospital IOP provides extended sta y treatment options for adolescents ages 12-17 that are not candidates for inpatient hospitalization but are not yet stable enough to return home or step back into intermittent outpatient sub-specialty care. These patients experience psychiatric symptoms and mood disorders, such as anxiety, depression, and injurious self-harm, with a degree of severity that prevents them participating in a full day of everyday life and activities. Patients will participate in programming 3 days a week, for 4-hour sessions in a combination of group therapy, family therapy, individual therapy, medication management, and 3 modalities of recreational therapy: art, music, and mind/body intervention. Patients will admit to the program for 6-8 weeks working towards goals based on individual care plans and will be reviewed bi-weekly in a treatment team conference to determine progression for discharge, or extended admission. Once patients complete their program, they will step down into on-going subspecialty care with psychiatry and/or psychology. The Intensive Outpatient Program, located in the Houston Medical Center will serve 40 patients every Monday, Wednesday, and Friday across four active cohorts grouped by ages 12-14 and 15-17. Tuesday and Thursdays are available for new patient intake appointments, post IOP follow up care with subspecialist, and Medication management with Psychiatrist. Our program expects 280 patients admitted annually over 26,850 patient visits.
