Christian Community Health Center (CCHC) is a community-based, African American led Federally Qualified Health Center (FQHC) on Chicago’s Far South Side (Roseland) and South Suburbs (South Holland, Calumet City and Lansing). For over 31 years, CCHC has provided providing quality, affordable primary health care, behavioral health services, co-located oral health care, HIV/AIDS-related testing and services, and the Ryan White Program, and permanent and interim supportive housing plus other specialty services, aimed at addressing social determinants of health plaguing some of the most disadvantaged, under resourced communities in the Chicagoland area. According to CCHC’s 2021 Community Health Needs Assessment (CHNA), diabetes was identified as a significant health issue in CCHC service areas. In the 2021 CHNA, a total of 13.8% of CCHC service area adults reported having been diagnosed with diabetes. Another 7.1% of the adults reported that they have been diagnosed with “pre-diabetes” or “borderline diabetes.” Diabetes is a condition that prevents the body from using and storing sugar properly. As a result, excessive amounts of sugar remain in the bloodstream and if uncontrolled, can cause damage to the tiny blood vessels all over the body, including those in your eyes. For this reason, eye doctors recommend annual eye exams for early detection of the disease and increased optimal treatment results.
