Community Project Funding/Congressionally Directed Spending - Construction - SUNY Downstate Health Sciences University will establish a community health primary care pilot site in Central Brooklyn that addresses health inequities and the complexity of our patients through a coordinated population health approach. Renovation of a new Family Medicine location that serves both children and adults will include technology that facilitates care management and assists with addressing social determinants of health.
